 The sliding fee scale is available for all of our services. Eligible patients are assigned a level depending on their family income. Each level has different fees that are much lower than Community of Hope's normal fees.

Hours: Monday - Friday 8:30 am - 5:00 pm.

Saturday 9:00 am - 3:30 pm.
