Hours: 

Monday, Wednesday and Friday: 8:00am - 5:00pm.

Tuesday and Thursday: 8:30am - 7:00pm.

Uninsured or underinsured patients are charged on a sliding fee scale that starts at $25 for an office visit. You must complete our Sliding Fee Scale Application and provide Proof of Income.
