If you do not have insurance, you give information about your household income. Which will us to determine the charge for your services. Sliding-fee scale is available upon request.

Monday – Friday: 8:00 am – 5:00 pm

Saturday: 9:00 am – 2:00 pm*

Sunday: CLOSED

*Only open the 1st & 3rd Saturday of the month
