Intake procedure: Must complete application and create Chart
Fees: Accepts Medicaid, Medicare, Private Pay Insurances and offers sliding scale fees.
Eligibility: Must be member of a federally recognized Tribe. There are exceptions to this requirement. Contact agency for details.
Documents required:To open a chart and begin services must bring Social Security card; Certificate of Degree of Indian Blood (CDIB); Photo ID; or Letter of descendency issued by a federally recognized Tribe; Current insurance information (Medicaid, Medicare, SoonerCare or private insurance)
Hours: Monday - Friday 7 AM - 5:30 PM
