Hours:  Monday - Friday 8:00 am - noon; 2:00 pm - 5:00 pm

Eligibility:

*Adults over 16

*Must be employed or spouse is employed

*Must not have any health insurance

*Must be an American citizen or have a green card

*Must have photo ID and proof of employment at each appointment
