Our experienced staff will work with you to confidentially determine the best health plan for which you are qualified.  If you do not meet eligibility standards for any reason, a sliding fee schedule is available.

Hours: Monday-Thursday: 8:00 a.m. – 7:00 p.m.

Friday: 8:00 a.m. – 5:00 p.m.
