DENTAL SERVICES PROVIDED
Dental services include dental x-rays, simple extractions, fillings and cleanings.
Teeth that are causing pain, broken or have decay are examples of what we may be able to treat at St. Martin's Healthcare. There are no charges for any service at St. Martin's, however a suggested donation of $5 for your screening visit and $10 for the visit with the dentist is greatly appreciated.
DENTAL SERVICES NOT PROVIDED
Work injuries, motor vehicle accidents are not treated at the clinic. We are not able to provide crown and bridge work, implants, orthodontics (braces), root canals, partials or dentures.

* Photo ID
* Proof of DeKalb County Residency
(A recent utility bill, rent receipt etc)
* Proof of Household Income for everyone living in the home
